What if you are unable to communicate your health care wishes? In Life Care Planning, you choose someone to speak about your health care wishes if there’s a time when you can’t speak for yourself.
Perhaps two of your most important choices are selecting someone to speak on your behalf and deciding on your health care wishes so that person knows what to say.
In this workshop, you will learn about how to select a health care agent, how to complete an Advance Health Care Directive form and get started with planning.
Ensure you receive the care YOU want to receive and ensure that your wishes are respected, known, acknowledged and part of your overall health care plan.
This workshop will be led by Sofia Padilla, a Clinical Health Educator at Kaiser Permanente San Francisco.
