Good Decisions-Living Life to the Fullest Until the Very End
Dying well takes planning. Do you need help getting started? Are you stuck? Join us to discuss what is important for you and get tools to get back on track to making good decisions for a good death.

This workshop will use the Compassion and Choices resource, My End of Life Decisions: An Advance Planning Guide and Toolkit, to discuss the values, beliefs, and wishes that form the basis for your end of life choices, and how to share them with your loved ones, friends and doctor.

We will then review a series of necessary documents that ensure your wishes will be known and carried out. These include an Advance Directive for Health Care, Dementia Provision, Physician Orders for Life-Sustaining Treatment (POLST), and Residential Agreement with Assisted Living Facility.

This session will be led by Regina Sneed, a Compassion & Choices volunteer who does presentations on issues related to the end of life.

NOTE: This event is held in a secure building (RSVP is required for entry).
