What is fun about discussing end-of-life wishes? Heart to Heart® Café is a card game designed to make it easier for a family member, a caregiver, or a health provider to understand what a loved one wants done when life is ending.
The Chinese American Coalition for Compassionate Care has designed a great activity to help start conversations, in Chinese or English, about end-of-life wishes. Heart to Heart® Café is a communication activity designed to make it easier for a family member, a caregiver, or a health provider to understand what a loved one wants done when life is ending.
Hamilton Madison House - Smith NORC and Knickerbocker Village NORC will be hosting a Heart to Heart® Café during its senior health fair on October 31st for older adults 60+. The health fair will also include free health screenings along with information and resources pertaining to health, nutrition and entitlement benefits.
