Heart to Heart Café (ACP Training & Workshop)
CACCC in collaboration with AACI, hosts a free online ACP Training & Workshop for Healthcare Professionals via the Heart to Heart® Café Workshop featuring Sandy Chen Stokes, RN, MSN.

The Chinese American Coalition for Compassionate Care (CACCC), in collaboration with AACI, hosts a free online Advance Care Planning Training & Workshop for Healthcare Professionals from 12:35-1:30pm.

• On November 8, CACCC founder Sandy Chen Stokes, RN, MSN, along with trained CACCC facilitators, will lead the Heart to Heart® Café Workshop where participants will use Heart to Heart® Cards to start the conversation.

Joining the Monday featured presenter on the panel of experts, during the Q&A session, will be representatives from CACCC, AACI and SCCVMC who will share their respective insights.

Who should attend: MD, RN, LVN, LMFT, LCSW, LPCC, LEP, Attorneys, Licensed Fiduciaries, Psychologists, and trained volunteers, and anyone interested in the topic.

*Must be present for the entire one (1) hour session; complete and submit program evaluation to receive 1.0 contact hour for one (1) CE per session. Santa Clara County Valley Medical Center is providing CE credits for RN, LVN (Provider #2056). The Institute on Aging is providing the following CE credits: CAMFT (Provider #135377 for LCSW/LMFT/LPCC/LEP) and State Bar of CA MCLE (Provider # 13264 for Attorneys/Fiduciaries).
