Curious or have questions about how to start/review your Advance Healthcare Plan? It’s so important there’s a whole National Day devoted to it. Jumpstart with us!

Advance Care Planning (ACP) is an umbrella term referring to the process of conversations & creating vital directives to express your healthcare goals, values, & wishes for when you’re not able to speak for yourself.

As part of National Healthcare Decision Day (US)/Advance Care Planning Day (Can.) on Friday, April 16,-Stefanie Elkins, Aging Life Care Manager® & End-of-Life Specialist, and Jennifer Timer, End of Life Doula & Grief Counsellor, will have fun being your ACP Co-Guides to help jumpstart your reflection and take action.

We added DATES: From Friday, April 16-Friday, April 23 from 9 am-3 pm PST, we are offering 20-min 1:1 friendly, consulting Zoom sessions to support where you are with your ACP.

After you RSVP( within 24 hrs), you’ll receive an email where you will schedule a day/time for your 1:1 and received other information to prepare for the session. We’ll be using The Conversation Project Starter Guide to help you start the conversation (and keep talking), so you can have a say in your healthcare — today and tomorrow.
