Conversation Starter Guides are tools to help you have a conversation with those who matter to you about your, or their, wishes regarding care through the end of life.

Talking with those you care about openly and honestly, before a medical crisis happens, gives everyone a shared understanding about what matters most to you through the end of life. You can use these Conversation Starter Guides whether you are getting ready to tell someone else what you want, or you want to help someone else get ready to share their wishes. We have several kits: to help choose a health care decision maker, to be a health care decision maker, to help caregivers of people living with Alzheimer’s disease or another form of dementia, to help talk with a health care team, and to help caregivers of seriously ill children. All are available in several languages are can be downloaded and printed for free. And, with the generous support from the Cambia Health Foundation, The Conversation Project and Ariadne Labs teamed up to create a new tool - Being Prepared in the Time of COVID-19 - to help people take action and be prepared. We can’t control how this pandemic plays out. But we can control who speaks for us if we’re unable to speak for ourselves, and we can take the time to make sure they know what matters most to us.
